Kickers – Fall 2023 – Boys

I agree to permit the above named minor to participate with the team assigned during the current season. I certify the date of birth and all other information is true and correct. I release VVYSA, Weber Complex, and any/all coaches from any and all liability for any injuries sustained during the athletic participation or while in transit to and from such facilities. We hereby agree the Soccer Association for Youth (SAY), its members, coaches, and officers shall not be liable for any injury or loss which my child or children may sustain while participating in activities of any kind whether sponsored by or under the supervision of SAY. And, we agree to indemnify and hold harmless SAY, its members, coaches, officers or designates of any kind from any claim whatsoever. I understand the VVYSA Code of Conduct (ABOVE), Lindsey Law (Sudden Cardiac Arrest) & Concussion Protocol (ABOVE).

In the event reasonable attempts to contact me have been unsuccessful, or if the need for medical attention is immediate, I hereby give my consent for the administration of any treatment deemed necessary and the transfer of said minor to the preferred hospital.
